South Nassau Communities Hospital Home Care has a Certified Home Care Program and a Long Term Home Health Care Program, also known as the Lombardi Program. This is a Medicaid funded program that offers coordinated case management services to patients who have chronic illnesses and would otherwise qualify from nursing home placement.These services include skilled nursing, physical, occupational and/or speech therapy, medical social work, registered dietician, home health aides, transportaion and PERS installation along with day care (both medical model and social) and home improvements when needed.
